Getting Chaos:

Take Vincent with you to the waterfall on the middle continent in disk 3. To get there, either take at least a green chocobo over the mountain or go by submarine and land on the rocky edges. Inside, a lady is there. She teaches Cloud about Vincent's past and her senses about Sephiroth. Afterwards, she'll have left you Vincent's final limit break.
